About our out-of-network model
-
As an out-of-network provider, we do not have a contract with your insurance company. This allows us to provide transparent, preventative, and flexible care to meet all of your needs without arbitrary visit limits or hidden costs. However, you may still be eligible for reimbursement from your insurance company. Please see below for more information.
-
Most of our clients receive at least partial reimbursement. We will give you a detailed “superbill” at the end of every visit. This provides you with the information required by your insurance company to submit for out-of-network reimbursement. Every insurance plan has a different process. Please contact your insurance company for details regarding requirements for reimbursement. Use this guide to help you through the process.
-
We believe in transparency and truly personalized care. Every person and situation is unique. Therefore, during the evaluation, you and your physical therapist with collaboratively develop your individualized plan, including frequency, duration, and estimated cost of services.
Under the No Surprises Act, if you do not have health insurance or do not plan to submit for insurance reimbursement, we will provide you with a Good Faith Estimate (GFE) of how much our services will likely cost. If you would like to request a GFE before scheduling, please call (978) 274-2090 or email us at sarah@live4pt.com.
