What is an out-of-network provider?

As an out-of-network provider, LIVE4 does not work directly with insurance. This allows us to provide transparent, preventative, and flexible care to meet all of your needs without arbitrary visit limits or hidden costs. You may be eligible for reimbursement from your insurance company. Please see below for more information.

Can I receive reimbursement from my insurance company?

At the end of every visit, you will receive a superbill. This provides you with the information required by your insurance company to submit for reimbursement. Every insurance plan has a different process. We recommend contacting your insurance company for details regarding requirements for reimbursement. Use this guide to help you through the process.

Do I need a referral?

The state of Massachusetts allows direct access to physical therapists. No referral is needed. If you plan to submit to insurance for reimbursement, please contact your insurance company to determine if a referral is required.

How do I schedule an appointment?

How do I use your client portal, PtEverywhere?

Watch this video to learn how to use our secure client portal. You will be able to independently schedule appointments, view invoices, send secure messages to your physical therapist, track your home exercise program, and more!

Can I receive a cost estimate?

At LIVE4, we believe in cost transparency and truly personalized care. A complete list of our prices can be found on our Services page. Every person and situation is unique. Therefore, during the evaluation, you and your physical therapist with collaboratively develop your individualized plan, including frequency, duration, and estimated cost of services.

Under the No Surprises Act, if you do not have health insurance or do not plan to submit for insurance reimbursement, we will provide you with a Good Faith Estimate (GFE) of how much our services will likely cost. Where are you located within West Acton Villageworks?

LIVE4 is a 1,500 sq ft open-concept clinic on the second floor of West Acton Villageworks (Unit 206B), to the right of the elevator and stairs. There is street level parking in the front of the building or additional parking underneath the building with elevator access available on all floors.
